Mayerson Center for Safe & Healthy Children

The Mayerson Center for Safe and Healthy Children (Center) provides services to abused and neglected children and their families. Located in the Sabin Education Building at Cincinnati Children’s Hospital Medical Center, the Center has state-of-the-art facilities for the forensic and medical evaluation of allegations of sexual abuse, severe physical abuse and medical neglect. In addition, the Center provides training programs for physicians, social workers, nurses, psychologists, psychiatrists and law enforcement personnel.

Web-Based Data System

Below are four attachments to be used by the Maternal Depression Project participants accessing the web-based data system.

1) Quickbase Add a Mom_demo standalone.exe- Tutorial on how to add a mom to the web-based data system. Watch this before adding any Moms to the test site. If you have not received an invitation thru Quickbase to log on to the -TESTING Ohio MD Project TESTING- web-based data system, Click here to send an email to Lisa Connelly requesting access.

Forms

For Home Visitors particpating in the Maternal Depression Project web-based data system. Print these two forms to use when screening Moms.

1) EPDS- Administer to Mom or have Mom complete the form herself. Keep this form in your records and enter the information into the web-based system.

2) Supporting Information Form- Use this form to capture the additional information needed for the web-based data system. This form is intended for Home Visitors to complete, not Moms.

Guide for Moms

"A Guide for Moms":

1) Helps mothers to think about their personal and family needs.

2) Gives mothers information about people, services, or resources that may help them through pregnancy and beyond.

3) Encourages mothers to seek help when theya re struggling or their families need help.

"A Guide for Moms: Resources for mothers-to-be and mothers of young children" was created at Case Western Reserve University under the direction of Amy Heneghan, MD with major funding from the Robert Wood Johnson Foundation.
